The calculation is a calendar-aware subtraction, not a simple year difference. Start with years = (reference year − birth year). If the reference date's month and day haven't yet reached the birth month and day, subtract one year, because the most recent birthday hasn't happened yet this cycle. The same logic applies one level down for months, and again for days — borrowing from the next unit up whenever the smaller unit would otherwise go negative.
For babies born prematurely, paediatricians and early-intervention services often track adjusted age (also called corrected age) alongside chronological age. Adjusted age subtracts the number of weeks a baby was born early from their chronological age, so developmental milestones are compared fairly against full-term babies rather than penalising a baby for arriving ahead of their due date.
