Why eGFR Is an Estimate, Not a Measurement

The "e" in eGFR stands for estimated, and the word is doing real work. True glomerular filtration rate is measured by tracking the clearance of an injected marker substance โ€” a procedure that is accurate, slow, and impractical for routine care.

What your blood test actually measures is creatinine, a waste product of muscle metabolism. An equation then converts that creatinine level into an estimated filtration rate using your age and sex.

This introduces a specific and predictable weakness. Because creatinine comes from muscle, anything that changes your muscle mass changes your creatinine independently of kidney function. A very muscular person can have a creatinine level that makes their kidneys look worse than they are. A person with low muscle mass โ€” including many older adults, and people with chronic illness or limb loss โ€” can have a creatinine level that makes their kidneys look better than they are.

Diet also affects it. A large cooked meat meal shortly before a blood test can transiently raise creatinine. Some medications alter creatinine secretion without affecting filtration at all.

What this means practically

A single eGFR result is a snapshot with real uncertainty around it. Kidney function is assessed on the trend across repeated tests, alongside urine tests for protein, and in the context of your overall health. One result slightly outside a reference range is a reason to repeat the test, not to draw conclusions.

Do not interpret your own eGFR result. Chronic kidney disease staging combines eGFR with albuminuria and clinical context. Your GP or a nephrologist should explain what your result means for you.

Sleep Cycle Calculators: What They Assume

Sleep cycle calculators work backwards from a single assumption: that sleep proceeds in cycles of roughly ninety minutes, and that waking at the end of a cycle rather than the middle of one leaves you feeling less groggy.

If you are consistently waking unrefreshed despite adequate time in bed, that is worth discussing with your GP. Persistent daytime sleepiness, loud snoring with pauses in breathing, or waking gasping can indicate a sleep disorder such as obstructive sleep apnoea, which is common, underdiagnosed, and treatable.

Body Surface Area: A Clinical Tool, Not a Personal Metric

Body surface area is used in clinical settings โ€” most notably in dosing certain chemotherapy agents, and in some cardiac and renal calculations โ€” because for those specific purposes it scales more appropriately than body weight alone.

It has essentially no application in everyday health, fitness, or weight management. There is no healthy or unhealthy BSA, no target value, and nothing useful to act on. The formulas (Du Bois, Mosteller, and others) exist to support clinical calculations performed by clinicians.
